The crypt on the right show the features of low grade dysplasia with enlarged crowded hyperchromatic nuclei
The dysplasia is low grade because there is not significant loss of polarity
The crypt on the left shows regenerative changes with the nuclei being round, having small nucleoli, less increase in nuclear chromatin, and maintaining a basal orientation
The distinction between dysplasia and regeneration can be difficult as shown in this image, and a diagnosis of dysplasia should only be made when it is clearcut
