"A" is apex; "D" is diaphragmatic surface.
Many enlarged airspaces are present in the both lobes, particularly in the upper portions (arrows).
The lower lobe also shows intense congestion and hemorrhage.
Air space enlargment does not involve all alveoli, hence not panacinar.
